TUE Application Form & Contacts

The TUE application form can be downloaded, filled out and sent to us.

• The TUE Application should be submitted at least 30 days before participating in an event.
• Please ensure that you submit ALL necessary information required on documents.
• Make use of the TUE Physicians Guidelines and Checklists for the various Medical Conditions – this will help to fast-track the application review process.
